So... how do you respawn?

As I type this I'm laying dead on the floor.  No respawn window or anything.  Do I have to log out every time I die?  If so, this game is going to be uninstalled pretty damned quickly.

Edit:  Found it finally in the wiki, had to search through the slash commands.  It seems pretty ridiculous that there's no "go to the hospital" or other respawn key or button.

I've always had a window when I've been KOed. There's two choices at the bottom: Call for Help (which is for if you're teamed, I think) and Recover.

Never got the window, but I bound the command to a key now.

Something is wrong if you aren't getting those options.  Maybe a setting somewhere?
